To cancel your benefits: 

  • To cancel your benefits you must have experienced a qualifying life event in the last 30 days and supporting documentation of the event must be provided.
  • You must submit an email to your manager requesting the cancellation of benefits along with proof of your life event. Please specify medical, dental, or vision.

  • Your manager will submit a ticket request that will notify the Benefits teams

  • Once the request is processed benefits coverage will cease at the end of the month. 

 

To Change Health Benefits: 

You may only make changes outside of new hire enrollment period or open enrollment if you experience a qualifying life eventYou have 30 days from the date of the qualifying life event to complete a life event declaration form via Dayforce.


1. Log into Dayforce


Note: If you need help logging into Dayforce, reach out to your manager. Once you log in, please set up your security questions to receive automated password resets in the future by using these instructions.


2. From the home screen, use the menu in the upper left corner and choose Profile & Settings



3. Select Forms and under the Benefits section select Life Event Declaration




4. Complete the Life Event Information form. You must provide proof of documentation by uploading the supporting documents via the life declaration form. (Birth certificate, divorce certificate, coverage documents) 


 


Once completed the Benefits team will review the submitted form and will approve or deny the request. If your request is approved, a new enrollment will open for you in the Benefits section of Dayforce.
