Basic Life and AD&D Insurance
Motivate provides an employer-paid Basic Life and AD&D benefit to eligible employees. The Life insurance benefit will be paid to your designated beneficiary in the event of death while covered under the plan. The AD&D benefit will be paid in the event of a loss of life or limb by accident while covered under the plan.
Optional Life and AD&D Insurance
You also have the option to purchase additional voluntary life insurance. However, your election amount could be subject to evidence of insurability. Elections for Optional Life and AD&D may be made independent of each other. If you would like to change your coverage at any time please reach out by submitting a PHD ticket.
Long-Term Disability Insurance (LTD)
Disability benefit premiums will be paid by Motivate. Disability benefits cover a portion of your salary when you are unable to work due to an accident or illness.

To check your coverage amounts:
1. Log in to Dayforce
Note: If you need help logging into Dayforce, reach out to your manager. Once you log in, please set up your security questions to receive automated password resets in the future by using these instructions.
2. From the home screen, use the menu in the upper left corner and choose Benefits 
2. Click on Current Elections where you will find details about all the benefits in which you are enrolled.
It's critical that all employees with benefits have a beneficiary recorded in Dayforce. Use these instructions to add or update your beneficiaries.
