When you depart employment from Motivate it's important to understand how your benefits will function moving forward. Below you will find information on what to expect in regards to Health Benefits, Life and AD&D Insurance, Flexible Spending Accounts, 401K, expense reimbursement, and Paid Time off (PTO and Sick Time off (STO).

Health Benefits (Medical, Dental, and Vision)
If applicable, any health benefits in which you were enrolled, including health care, dental care, and vision, terminate on the last day of the final month of your employment.
As a result, Motivate will deduct your full employee contribution for your medical, dental and vision deductions on your final paycheck with Motivate. Motivate will not prorate any health benefits contributions on your final paycheck.
Consolidated Omnibus Budget Reconciliation Act (COBRA)
When employment terminates, the Consolidated Omnibus Budget Reconciliation Act (COBRA) allows you to extend your medical and dental and vision coverages at your own expense.
Our COBRA administrator is Optum Financial. Within 30 days of the conclusion of your coverage, Optum Financial will notify you of your rights and the steps you must follow to continue your Health Benefits. For your enrollment to be processed, Optum Financial must receive all paperwork and payments by the deadlines listed in your COBRA notification package.

Life Insurance, Accidental Death and Dismemberment (AD&D)
Your Life and AD&D coverage will terminate on the last day of your employment. Motivate will not prorate any Life or AD&D contributions on your final paycheck.
If you would like to continue your Life or AD&D benefits after your employment with Motivate ends, you may be able to port or convert your coverage. You have 31 days after your termination date to port or convert your coverage. If you have questions about your options, please contact Sunlife at 877-736-4739.
Flexible Spending (FSA), Dependent Care Accounts, Commuter/Transit, and Parking
Your FSA, Dependent Care, Commuter/Transit, and Parking benefits will terminate at the end of the final month of your employment. You will have FSA, Dependent Care, Commuter/Transit, and Parking contributions deducted from your final paycheck. Motivate will not prorate any of these deductions from your final paycheck.
You have 30 days after your termination date to submit all your expenses incurred this year to Cigna for reimbursement. These claims must have been incurred while you were an active employee at Motivate. Any unused funds after the end of this 30-day period will be forfeited and will be unrecoverable. If you have questions about your account, contact Cigna at 800-244-6442.
401(k)
Your 401k eligibility terminates on your termination date. Your final paycheck will include a 401(k) deduction and a 401(k) match. To request a 401(k) rollover or if you have questions about your account, please contact Fidelity at 1-800-343-3548.
Expense Reimbursement(s)
Any final expense reimbursements must be submitted to your manager for approval by the last day of the final month of your employment.
Paid Time Off (PTO) / Sick
PTO will be paid out in accordance with the terms provided in Motivate's employee handbook. Sick time is not paid out.
