Motivate encourages all employees to get vaccinated as soon as possible. Motivate does not require proof of vaccination to enter a Motivate facility.
Through September 30, 2021, eligible Motivate employees may take paid leave to receive the COVID-19 vaccine off-premises. An eligible employee (i.e. all full-time, part-time, seasonal, and probationary employees) may request up to two (2) hours of paid leave for each dosage of the vaccine administered during this period. Eligible employees must provide as much notice as possible to their manager to ensure both adequate coverage and proper payroll treatment for the time off. Employees may be asked to provide medical documentation of the vaccination appointment.
New York Residents and Employees: You have additional leave available to use for receiving each dose of the vaccine up to a maximum of four (4) hours per dose.
Vaccination leave will be paid on the first payroll of the month for the previous months requests. Please submit a ticket for leave to be applied to an employee's timesheet.
Please note, you will need to follow these instructions to upload your vaccine card if required (D.C. & NYC Only). This is required to be completed separately. Submitting for vaccine leave does not suffice "in lieu" of uploading vaccine card. You are still required to upload vaccine card once you have received your dose(s).
