Mutual of Omaha is where Motivate employees and managers can request and monitor the different types of leaves of absence available at Motivate, such as those related to FMLA What is FMLA?, Americans with Disabilities Act (ADA) and Personal Leave of Absence.



Please note: If applicable, you are responsible for any insurance premiums (healthcare or voluntary) while on your leave of absence. Upon return from your leave of absence, your missed premiums will be collected through payroll over your next 6 pay periods. Getting Started 


1. Accessing Mutual of Omaha


Visit Mutual of Omaha website via any internet browser and you'll be taken to the homepage below. If it is your first time using Mutual of Omaha, you will need to register an account. DO NOT use your motivate email address to receive correspondence from Mutual of Omaha. 
 

 

 

2. Register an Account 

You will need your employee number (that starts with M00) in order to complete the registration. If you are unsure of your employee number, try the number you use to clock in at work or follow the instructions in this PHD article. 

If these suggestions do not work, please contact or email Mutual of Omaha If they are unable to assist, they will work with the Benefits administrator to resolve the issue. 

For Postal Code, please enter your home zip code. Reminder: DO NOT use your motivate email address to receive correspondence from Mutual of Omaha.  
 

 


 

3. Continue Registering Your Account 


Choose a username and password for your account. Proceed to select 5 security questions.   

 
 
 

4. Finish Registering Your Account 


Correspondence will include notifications about decisions and updates on your claim.  

Auto-notifications will be brief reminders about paperwork due dates and claim expiration.  

 
Text notifications will advise of when we receive paperwork for your claim. Once everything is complete, click “submit”. 


 

And with that, you're done!
