1. Access Mutual of Omaha via any of the methods below.

2. Initiating a Leave Request


Mutual of Omaha will then reach out to the employee to explain leave process and company policies. A leave request case will be opened in our system. This is done to determine eligibility for FMLA or other leave. They will seek authorization to contact the health care provider(s) if necessary. In addition, Mutual of Omaha will perform short term disability (STD) intake, if applicable. 

 

3. Paperwork Sent 


Within 5 days of the request, Mutual of Omaha will send the following: 

  • Leave “request letter” 
  • “Your Next Steps” document 
  • Required rights and responsibilities documents 
  • The blank medical certification form 
  • Any applicable return to work documentation 
  • Mutual of Omaha will notify employees by phone or email of approaching deadlines 
     

4. Paperwork Returned 


Employee or health care provider must return completed paperwork. This is typically due 15 days after issuance, plus your grace period. View additional information below: 

  • Incomplete paperwork — additional 7 days to “cure” 
  • Mutual of Omaha will conduct outreach to health care providers on behalf of the employee if authorized by the employee 
  • Mutual of Omaha will notify employees by phone or email anytime paperwork is received 

 

5. Decision Rendered 


Mutual of Omaha may send the employee the following documentation: 

  • “Decision letters” for the leave and STD requests indicating approval, denial, or partial approval 
  • “Your next steps” document 
  • Any applicable return to work documentation 
  • Benefits Admin and the employee's manager will be notified of the leave decision and any approved dates via work email 


6. Insurance Premiums 


If applicable, you are responsible for any insurance premiums (healthcare or voluntary) while on your leave of absence. Upon return from your leave of absence, your missed premiums will be collected through payroll over your next 6 pay periods.
