As more Americans are getting vaccinated against COVID-19, health and safety in our workplaces is increasing, and we may be able to consider options that were unavailable throughout the COVID-19 pandemic.
That journey starts with understanding who is fully vaccinated and who is not. In order to do that, we need each member of the Motivate team to let us know their vaccination status and share their vaccine documentation with us to ensure we have sufficient records to make the right decisions for the larger team’s continued safety. Fortunately, you can add your vaccine information into an app many of us are already very familiar with: Dayforce.
Follow the steps below to upload your vaccine documentation into Dayforce:
- Log in to Dayforce either on a computer or mobile device. If you have multiple roles in Dayforce, make sure you are in the Hourly Employee or Salaried Employee role.
Note: If you need help logging into Dayforce, reach out to your manager. Once you log in, please set up your security questions to receive automated password resets in the future by using these instructions. - Find your Profile by clicking Profile underneath your name at the top left, or by finding the Profile option in the main menu accessible via the icon. or
- On the Profile page, click the Forms tab
- On the Forms page, scroll down to find the COVID-19 section with the form Covid Vaccine. Click to open the form
- Using the information from your vaccination card (example below) type in the date when you received a COVID-19 vaccine for either your single dose (for Johnson & Johnson) or for both doses (for Pfizer or Moderna), and indicate which type of vaccine by typing in the name in the field provided.
- Use the Upload Files button to then upload a photo of your vaccine card to Dayforce. Please note, "PDF" format is the most compatible format to use when uploading your vaccine card document, JPEG format will also work.
- Once you have finished, click the Submit button.
